# Velarion Records — full reference > The 340B register and hospital finances — joined, watched daily, delivered. > Public registries, as they were filed. Every figure on this site is the value its source file printed, shown with the name of that file and the date its publisher stamped it. > This is the expanded companion to /llms.txt: the same map with a paragraph > on every class of record and every product. Record pages are listed in > /llms.txt, not repeated here — each one already states its own figures with > the file they came from. ## Data ### 340B records by state Every 340B covered entity, by state. A record is one 340B identifier, not one hospital: a parent registration and each site registered beneath it are separate records, and every state links all of them. The register is the HRSA 340B OPAIS Covered Entity Daily Report, published by the Health Resources and Services Administration, and each record page shows the entity type, the participating status the report prints, and the changes each day’s report brought. URL: https://velarionrecords.com/340b ### Hospitals by state Hospitals that filed a Medicare cost report, with the figures each worksheet prints. The finances are the Medicare cost report, form CMS-2552-10, published by the Centers for Medicare & Medicaid Services; every figure taken from one names its worksheet, line and column. The cost reports are joined to the 340B register on the certification number both documents print, never on a resemblance between names. URL: https://velarionrecords.com/hospitals ### Nursing facilities by state Every published nursing facility, by state: Medicare cost reports figure by figure, inspections, deficiencies, penalties and ownership as filed with CMS, by state or in one table you order. The records come from CMS Care Compare and from the cost reports the facilities file, and every figure is printed as its source file printed it, beside the name of that file and the date its publisher stamped inside it. URL: https://velarionrecords.com/facilities ### Clean Water Act discharge permits Anyone who discharges to a river, lake or coastal water in the United States holds an NPDES permit: sewage treatment plants, factories, livestock operations, mines, municipal storm sewers and construction sites. These pages group the current register by state, by city, by the watershed the discharge reaches, and by the kind of operation permitted. The register is the National Pollutant Discharge Elimination System permit register, kept under the Clean Water Act. URL: https://velarionrecords.com/npdes ### Hazardous waste handlers Every site EPA tracks as a regulated hazardous waste handler — the generators that produce the waste, the transporters that move it, and the facilities licensed to treat, store or dispose of it — with the county it sits in, the industry it belongs to, and its inspection and violation record. The register is RCRAInfo, kept under the Resource Conservation and Recovery Act. URL: https://velarionrecords.com/rcra ### EPA air-emissions sources Every facility EPA’s Integrated Compliance Information System for Air tracks as a stationary source — from large industrial plants to dry cleaners — with its state, county, primary industry code, and its Clean Air Act emissions classification. The register is ICIS-AIR, kept under the Clean Air Act. URL: https://velarionrecords.com/epaair ### FMCSA motor carriers by state Every active interstate and intrastate-for-hire motor carrier registers with FMCSA on the Company Census File (MCS-150). These pages group that register by county, cargo carried, operating authority, fleet size, driver count and safety rating. URL: https://velarionrecords.com/fmcsa ### 340B contract pharmacies One registered contract pharmacy, and the covered entities that send 340B prescriptions to it. The covered entity’s own page lists the pharmacies it holds arrangements with; this page is the reverse of that table, answering which covered entities a single pharmacy serves. There is no index page for the class — the address above is the shape of a record, and each pharmacy is printed as the HRSA Covered Entity Daily Report shows it. URL: https://velarionrecords.com/pharmacy/{id} ### Hazmat transport and generation One page per county, the carriers that move hazardous material and the generators of it shown side by side. The two programs are collected by different agencies under different statutes and are shown together here because they describe the same place. There is no index page for the class — the address above is the shape of a county page. URL: https://velarionrecords.com/hazmat/county/{state}/{slug} ## Products ### Record report The citable document for one hospital, or for one 340B record: its filed figures with the cell each came from, set beside five hospitals of its own size and against its own history, in a signed, dated PDF you can put in a board pack, a memo or a finding. Every figure is printed as its source document printed it, beside the name of that file and the date its publisher stamped inside it. Both registers sell the same document, so both doors show the same price. Price: $295 URL: https://velarionrecords.com/hospital/{ccn}/report · https://velarionrecords.com/340b/{id}/report ### Post-acute record report The citable document with a nursing facility’s full history, as it stands on the day you buy it: every cost report on file figure by figure, with the worksheet, line and column of form CMS-2540 each was read from; the facility beside the facilities of a similar size in the same state at the same fiscal year; ownership from both CMS ownership files; every inspection, deficiency and penalty on file; and nurse staffing quarter by quarter, on a page of its own. Price: $295 URL: https://velarionrecords.com/facility/{ccn}/report ### Register change alerts The register is republished every morning as a fresh spreadsheet with no record of what moved. These alerts are that missing record: one email a day naming what changed on the entities or the contract pharmacies you follow, each line with the value before, the value after, and the report that first showed it. A pharmacy is followed by its own register ID, so its arrangements with every covered entity arrive in one mail, whichever hospital they are with. Price: one hospital $99/month · one system $2,990/year · one state $4,990/year · everything $18,000/year URL: https://velarionrecords.com/alerts ### Post-acute alerts A line each time a cost report is filed or a figure on one moves, a quarter’s nurse staffing falls at least 10% below the quarter before, a provider detail changes, or an owner is listed or removed on a skilled nursing facility record — and each new inspection, deficiency or penalty as CMS publishes it. One facility, or a portfolio you name. Price: one facility $99/month · a portfolio of facilities you name $6,000/year URL: https://velarionrecords.com/alerts ### Data license The public source is a spreadsheet that is replaced every morning and keeps no history, so the answer to “when did this arrangement start” is only as good as the day somebody on your team began saving copies. This is the same register with its history kept for you: every entity, every site, every dispensing arrangement, every value that changed and the report that first showed it — joined to the Medicare cost report of each hospital that files one. A one-year term from the day it is paid; the file and the API key arrive by email within minutes, with the license itself as a PDF naming the scope, the term and what you may do with the data. Price: one state $4,500/year · the whole register, internal use $25,000/year · enterprise, with redistribution $75,000/year URL: https://velarionrecords.com/license ### Post-acute data license Every figure a nursing facility page shows, for one state or every loaded facility: finances, staffing, inspections, deficiencies, penalties and ownership. Price: one state $4,500/year · every loaded facility $15,000/year URL: https://velarionrecords.com/license ### Custom research A written study of one state or one health system, taking the register and the cost reports as far as they go on a question you set. The scope is agreed and published before the work starts, the finished study is published on this site, and the same scope is offered to anyone on either side of the question at the same price. Price: from $15,000 URL: https://velarionrecords.com/license ### Hazardous waste handlers report RCRAInfo is the federal register of hazardous waste handlers kept under the Resource Conservation and Recovery Act. That file is history, county by county and cause by cause, and the agency gives it away. What no source publishes is the county-crop's own band — the range its annual loss ratio has actually run, read against the causes that produced it. That band is the join, and it is what is sold here: one county and one commodity, drawn from the agency's whole filed record and published as a dated document on the day of purchase, every figure carrying the filing it came from. Price: priced on request URL: https://velarionrecords.com/croploss/report ### Crop-loss bands by county alerts The Risk Management Agency files, for every insured county and commodity in the United States, the premium, the indemnity, the liability, the quantity planted and lost, and the policies that filed a loss.
