Velarion Records republishes two public federal documents as pages you can read, search and cite: the register of organizations taking part in the 340B program, and the Medicare cost reports the hospitals among them file. Every figure is printed as its source document printed it, beside the name of that file and the date its publisher stamped inside it.
What is here
94,363 records on the federal 340B register, of which 64,413 are shown as currently taking part, the pharmacies those organizations contract with, and the finances of the hospitals that file a Medicare cost report — the two joined on the certification number both documents print, never on a resemblance between names. The register pages are built from the report of 7 September 2026.
Why it exists
The 340B register is republished every day as a fresh snapshot with no record of what changed. Yesterday’s file is gone when today’s arrives, so a hospital cannot show what its own listing said last month, and nobody can say what moved without having kept both files. Keeping them, and printing the difference, is most of what this site does.
What we will not do
- Take a side. Velarion Records has no position on the 340B program, its rules, or anyone taking part in it. The figures are the record and the reader draws the conclusions.
- Estimate. Where a filing prints a value, the page prints the same value. Where it prints nothing, the page shows nothing — no estimate, and no figure carried in from another year or another document to fill the space.
- Rank anyone. Where a page gives a position among comparable hospitals, that is a position and not a score. Hospitals are counted, never ranked, and no word is attached to where one sits.
- Charge for the record. Every page on this site is free and stays free, including every figure on it. What is sold is the daily change email, the record as a document, and the data under license — never access to what a public file says.
When we are wrong
Every page names the file it was read from and the date stamped inside it, which is what makes an error checkable rather than arguable. If a figure here does not match its source, write to us with the 340B identifier: what we have corrected, and when, is on Corrections. A record that is wrong at the source is fixed with the publisher, and the fix appears here as a change with its own date.

The documents
The register is the HRSA 340B OPAIS Covered Entity Daily Report, published by the Health Resources and Services Administration. The hospital finances are the Medicare cost report, form CMS-2552-10, published by the Centers for Medicare & Medicaid Services; every figure taken from one names its worksheet, line and column.
